Affiliation Forms and Rules

Ok I'm going to start off with our rules about affiliating.
(Note: More rules can be added, changed, or taken at anytime)
Rules:

All Affiliates or interested parties must have out banner on their page before we add theirs.
There is no post an affiliation post and then say you will wait to put up our banner until you are accepted.
Any site we use may/will have a banner without a form
All roleplaying or image sites must use the correct form.
All rejected sites have about 10 days before you must file a new post/form.
All forms are to be in their own posts.
